Adoption Application

We ask that you review the Adoption FAQ’s prior to submitting your application.  It is important that you understand GRRAND’s adoption process and requirements, some of which may impact the suitability of your application (geographic areas served by GRRAND, fencing requirements, etc.).

As you respond to the application questions please keep in mind that some of these dogs may have been in neglectful and/or abusive situations and will require your patience as they adjust to a new home.  In addition, the average Golden’s lifespan is 10—14 years and it is important that you understand that adoption is a long-term commitment with regard to training, medical treatment and proper care.

Home Visit during Covid:  GRRAND will contact you about a home visit and where possible do an onsite visit on a patio or porch with social distancing and masks.  If you live in an apartment with no access to an outside area we will meet somewhere outside in an area near the apartment or as a last resort, meet via Facetime.

At this time we are unable to process any applications that are outside of the geographic areas listed below:

  • Greater Louisville, KY
  • Greater Lexington, KY
  • Ohio Counties:  Hamilton, Butler, Warren, some areas within Clermont county, Montgomery and Green 
  • Northern KY Counties: Boone, Kenton and Campbell

Privacy Statement: GRRAND will not share your personal information with other individuals or organizations without your permission.  The only exception is if disclosure is necessary to comply with the law or a court order compels us to. We will share your information with government agencies if they need or request it.

A response to each question on this application is required. If you omit a question, the application will not be processed when you select “submit.” In this case, browse through the application and look for the highlighted errors, make your corrections and then re-submit.

Please note: Your complete address is required in order to process your application.

What interests/hobbies do you enjoy?
Example: 9 hours a day while at work, 7 hours a week for a recurring commitment, few hours to run errands, etc.
Provide detailed information about the type and frequency of exercise you will give your dog on a regular basis.
Please respond "Yes" if you have any type of fence that will restrict a dog to your property, INCLUDING UNDERGROUND fencing.
If you own any other pets, please identify the type of pet, breed (if applicable), age, and name. If you do not own any other pets please input, "NA."
In the past 6 years, how many dogs have you owned that you no longer have?
In the past 6 years, how many cats have you owned that you no longer have?
Please describe these dogs and/or cats, and explain what happened to them.
If you answered "Yes" to this question, enter all requested veterinary clinic information for your current or previous dog or cat. We cannot process your application without this information. If you don't have a pet, enter the information for your previous dog or cat. Do not list the name of a vet that you plan to use in the future.
To expedite your application please provide responses for all fields below and notify your references that GRRAND will be calling. Please provide 3 references that are:
  • not household members,
  • not your veterinarian, and
  • only one can be a relative
